A young woman, braving the challenges of life in Kabul, found herself facing a formidable opponent - a C2 Glomus Jugulare Tumor. This intricate mass, nestled deep within the skull base near the ear, had begun to encroach on the facial nerve, robbing her of the ability to smile or express emotions effortlessly i.e., facial nerve paralysis. In this critical moment, she sought the expertise of Dr. Abdul Bari and Dr. Akbar Abbas, two accomplished surgeons at Aga Khan University Hospital in Karachi, Pakistan.



The tumor, classified as "C2" based on its location, presented a particularly challenging scenario. It nestled close to vital structures like the sigmoid sinus (a major blood vessel draining the brain) and the jugular bulb (part of the jugular vein). Dr. Bari and Dr. Abbas embarked on a meticulously planned operation, employing the Fisch Type A approach. This highly intricate technique involved creating a wide surgical window, providing a clear view of the tumor and surrounding structures. The sigmoid sinus unfortunately had to be obliterated (blocked) due to the tumor's aggressive nature, necessitating rerouting of blood flow through an alternative pathway.



With utmost precision, the surgeons then tackled the crucial task of separating the internal carotid artery, responsible for supplying blood to the brain, from the tumor itself. This delicate dance required advanced techniques like Jugulare Tumor resection and neuro-navigation, a technology utilizing real-time imaging to guide surgical instruments with pinpoint accuracy.



The ultimate goal was maximal safe tumor removal. While complete removal wasn't always possible, maximizing tumor resection without compromising neighboring nerves and blood vessels was paramount. To promote healing and minimize complications, the surgeons employed a clever technique. They harvested autologous fat and fascia (tissue) from the patient's own abdomen and thigh, carefully filling the cavity left behind by the tumor. This not only filled the space but also promoted tissue growth and reduced the risk of infection.



The surgery, a testament to the surgeons' skill and meticulous planning, proved successful. The young woman, her facial nerve function gradually recovering, was discharged home within a week.
